Responsible for the full client care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients’ health and human services needs. Under minimal supervision, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. Maintain a caseload of medication management clients, necessitating quarterly contact and occasional community visits. This is primarily an office-based position. The role involves handling case assignments, drafting service plans, reviewing case progress, and determining case closure for review and/or in consult with a supervisor. The Case Manager delivers ongoing services to clients which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral and linkage to community resources as needed. They assist clients in achieving wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.). The position requires developing and/or implementing individualized client treatment or service plans and ensuring compliance with applicable standards and agency policies. All clinical activities must be documented timely and with accuracy to meet specific program, funder, and credentialing entity criteria. The Case Manager participates in treatment team meetings and assists in making recommendations for client status changes. They may also assist in providing education and/or group curriculum and must adhere to professional standards as outlined by protocols, rules, and regulations. Completion of agency, accrediting body, and funder required training is also necessary.
