Zepf Center has been serving the Lucas County community for over 50 years and is the leading provider of behavioral health and substance use disorder services in Northwest Ohio. This position's purpose is to provide a wide range of community support and coordination of services to people experiencing serious mental illness and/or addiction challenges to ensure their needs are addressed. The Case Manager supports clients in the development of their short and long-term goals and acquisition of community resources using a person-centered approach. Services will be provided with respect to cultural preferences and in accordance with client’s rights. The person in this position participates in the development of comprehensive care coordination and treatment planning, advocates for persons on their caseload, utilizes evidence-based assessments, provides linkage and referral to additional community resources as needed, coordinates crisis intervention services and generally supports the client in their pursuit of emotional and physical health and wellness. Levels of support required by people on the caseload may vary greatly. The Case Manager works with the client in the community, home environment, and at agency locations.
