Provides support and instruction to tenants in areas such as financial literacy, computer skills, healthy meals and snacks, kitchen safety, exercises for health and wellness, and other educational programs geared toward the frail/elderly population. Assists the Service Coordinator in developing and implementing person-centered service plans. Ensures the health and safety of each tenant, providing enhanced support for high-risk individuals. Completes daily documentation and paperwork promptly and accurately. Participates in staff meetings, in-service training (including incident reporting, crisis intervention, and productive resident interaction), and other agency meetings/trainings. Works collaboratively with the Service Coordinator/Manager and team members. Records time worked accurately and works the minimum number of accepted hours. Safely transports people to and from activities when approved to drive, following proper procedures. Ensures adequate supplies are available to support persons served. Complies with all agency policies and procedures. Other duties as assigned.
