About The Position

The CAPCHP is responsible for managing the daily operations of the Community Action Plan for Prenatal Child Health Program (CAPCHP) within an Anishinabe Health and Wellness Framework. The CAPCHP worker will be accountable to the Program Manager or designate for the proper completion of the functions outlined in the job description. The CAPCHP worker usually work in group settings and an office, but the mission of the organization may sometimes take them to non-standard workplaces. The CAPCHP worker works a flexible 35-hour work week; Monday to Friday. All employees are required to carry out their duties in a manner that does not adversely affect their own health and safety and that of other by reporting all potential hazards; and all incidents and injuries; as well as co-operating with any measure introduced in the workplace to improve OH&S.

Requirements

  • Diploma or Degree in Social Work, Nursing, Health Sciences, or Human Services.
  • Experience in prenatal/postnatal care and/or child development.
  • Knowledge of Indigenous health issues
  • Strong organizational and communication skills.
  • Providing a successful Police Records Check (Vulnerable Sector)
  • Must be willing to work flexible hours of work (35 hrs/wk)
  • Must have a valid Ontario Class “G” Driver’s license, use of an automobile and appropriate insurance coverage is required
  • Must sign Oath of Confidentiality
  • Must have First Aid CPR & AED Level C Certification
  • Must have Safe Food Handling Certification

  • Experience working in an Indigenous organization.
  • Ability to speak Ojibway, Oji-Cree or Cree.

Responsibilities

  • Work from a holistic perspective, utilizing the teachings of the medicine wheel and Seven Grandfather Teachings.
  • Utilize Elders for teachings, healing and guidance.
  • Take personal responsibility to increase sensitivity, awareness and implementation of Beendigen’s cultural teachings and organizational practices in both professional conduct and work-related deliverables.
  • Coordinate project activities in conjunction with existing community programs and facilities that meet Program goals and objectives.
  • Plan, implement, and evaluate prenatal, postnatal, and early childhood programs
  • Deliver prenatal education, nutrition counselling, breastfeeding support, parenting programs, and prevention programming
  • Provide direct support through home visits, hospital visits, and community-based programming
  • Coordinate program activities with community services and facilities
  • Provide individual and group support to families
  • Support high-risk population
  • Assist families in achieving personal and wellness goals
  • Provide referrals and system navigation
  • Deliver services using the Medicine Wheel and Seven Grandfather Teachings
  • Engage Elders for guidance and healing
  • Build and maintain community partnerships
  • Promote culturally grounded programming
  • Maintain program statistics and reports
  • Ensure compliance with funding agreements
  • Develop proposals and program policies
  • Ensure availability of materials and equipment
  • Arrange transportation for participants
  • Maintain safe program environments

  • Promote the organization
  • Promotes Beendigen at all times, when networking and events.

  • Representation on committees as required for the enhancement and benefits of Beendigens programs.

  • The completion of functions outlined in the description and the achievement of goals set to a high level.
  • Regular attendance and conduct at work according to the requirements of the Beendigens Policies and Procedures.
  • Any other duties as requested by the Program Manager or designate, commensurate with role.
