About The Position

Shared Health leads the planning and coordinates the integration of patient-centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations. Position OverviewThe Shared Health Mental Health and Addictions program provides a range of mental health and addiction services across the healthcare continuum that support and promote recovery. Through collaboration and partnership across various health, social service and community sectors the program strives to promote mental health and well-being for all, to reduce inequities and the impact of mental health and addiction problems and to work towards sustainability of mentally healthy individuals and communities. To provide assertive, long term, flexible clinical counselling interventions and case management services to adults with severe and/or persistent mental illness co-occurring with substance use disorders. As well, to provide expert assessment of mental health and substance use disorders and to consult to other service providers across the system, including primary care physicians, and clinicians from other agencies, to assist them with treatment planning for their clients. The focus of assessments, interventions and consultations is rehabilitative in nature. In addition to rehabilitative services, the incumbent will work to provide and coordinate a range of services including: specialized mental health and addictions treatment; crisis response service; general health, housing, income support, protection and advocacy; client outreach services; and general support to family, peer and community networks. As part of a clinical/academic program, collection of clinical data to support outcomes-based research is a key function. The case manager will be required to function as an autonomous mental health expert within the team setting.

Requirements

  • 4 years relevant work experience.
  • Bachelor degree in a relevant human services/health field (e.g., BSW, BSc MH, BSc, BN, OT, RPN, RN.).
  • Licensure/Registration as per professional designation required.
  • Valid driver’s license and access to use of a vehicle.
  • Interest in working with people with co-occurring mental health and substance use disorder.
  • Excellent interpersonal skills.
  • Ability to work in a community care setting as a member of a multidisciplinary team serving a population with serious and persistent mental illnesses and co-occurring substance use disorders.
  • Demonstrated ability to function creatively and independently.
  • Current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check.

  • Clinical Master’s degree preferred.
