About The Position

To provide personal care and comfort services to terminally ill patients, regardless of their age, and perform related homemaker tasks in the patient/family residences or Cottingham Hospice House, in accordance with the plan of care, as assigned and supervised by the registered nurse.

Requirements

  • Completion of a nursing assistant or patient care tech program
  • CNA - NURSING ASSISTANT - NCNA
  • Basic computer skills including word processing and data entry
  • Knowledge of office equipment (fax/copier)
  • Mathematical skills
  • If driving is required for the position, must provide a valid driver’s license, a certificate of auto insurance and have an acceptable motor vehicle record.

Nice To Haves

  • In lieu of school verification of a nursing assistant training course, will accept current or former South Carolina CNA certification as proof of completed state required training at time of hire.

Responsibilities

  • Personal care and comfort: Bathes patient, assists/provides mouth care, shampoos hair, performs basic nail and foot care, shaves patient, assists patient to dress, assists with ambulation and transfers, turns and positions patient, assists patient with range of motion exercises, facilitates elimination, facilitates nutrition, models competent personal care techniques and communication skills, performs tasks for all ages/developmental stages, participates in bereavement care.
  • Monitor Patient's condition: Assists the nurse by monitoring the patient's condition and reporting any changes, measures and records vital signs, measures I&O, observes and reports changes in skin color or tears, monitors non-sterile dressings and changes as directed by RN, reports difficulties with elimination, reports changes in patient's mental status, utilizes communication and listening skills, observes and reports patient compliance with safety measures, participates in team conference and IDG, reports changes in patient's physical strength, endurance or coordination, is knowledgeable in issues related to death and grieving.
  • Homemaker Tasks: Performs basic housekeeping duties including changing linen, laundering, washing dishes, cleaning kitchen counters, emptying out of date food, organizing food and supplies, delivering medical supplies, removing clutter to reduce fall risk, cleaning and sanitizing durable medical equipment and bathroom area, running errands.
  • Agency Needs: Is thoroughly familiar with department policies and procedures and demonstrates compliance with hospice standards of care, including infection control and bloodborne pathogens. Participates in review of QI data and makes appropriate recommendations for improvement. Manages agency resources by demonstrating reasonable and efficient use of time, mileage, visit schedules and pre-visit activity. Monitors utilization of medical supplies, maintains adequate supply in the home. Assists in organizing and cleaning storeroom. Assists with office duties as requested.
