About The Position

This is a Part-time CNA position on the Telemetry: 2 East unit at Advocate South Suburban Hospital. The schedule is for 24 hours per week, with shifts from 7 PM to 7 AM. The role involves providing direct patient care, ensuring customer satisfaction, maintaining a safe and clean environment, and supporting the nursing staff.

Requirements

  • High school graduate or equivalent.
  • Ability to read, write and follow instructions as required.
  • Good communication skills.
  • Courteous and professional interpersonal skills.
  • Basic math calculation skills.
  • CPR (Heart Saver) certified within 6 months of employment.
  • Certification as a Nursing Assistant by the State of Illinois.
  • Ability to communicate with customers.
  • Ability to push and pull equipment associated with duties.
  • Ability to do medium lifting (20 to 50 pounds occasionally and/or 10 to 25 pounds frequently and/or up to 10 pounds constantly).
  • Ability to lift up to 35 pounds without assistance.
  • For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available.

Responsibilities

  • Introduces self and role to patient at start of shift.
  • Provides fresh pitcher of water (if not NPO).
  • Places call light and phone in reach of the patient.
  • Listens to patients and solicits information about their needs.
  • Involves patients in decisions about their care.
  • Answers call lights.
  • Assists patients with elimination needs.
  • Provides a.m. or p.m. care including bathing assistance, oral care, and hair care.
  • Straightens patient’s room and changes bed linens.
  • Transports, transfers, lifts, and positions patients as directed.
  • Ambulates non-bed ridden patients.
  • Feeds patients as needed.
  • Collects and transports specimens to the lab.
  • Transports equipment, supplies, and pharmacy items.
  • Communicates with patients, families, and other health care professionals in a positive and respectful manner.
  • Addresses and resolves customer complaints.
  • Ensures on-time completion of performance pathway.
  • Sends out co-worker feedback forms.
  • Submits self-evaluation to leader.
  • Participates in the TNT associate satisfaction survey.
  • Submits ideas for improving customer satisfaction.
  • Receives report from previous assistant at start of shift.
  • Prepares NPO list and hangs NPO sign above patient’s bed (Night shift).
  • Communicates with patient in layperson’s terms.
  • Participates in the admission, transfer, and discharge process of all patients under the supervision of the registered nurse.
  • Sets up patient room, obtains necessary supplies and equipment.
  • Introduces self to patient and family; assists patient in changing into a patient gown.
  • Ensures patient identification band placement.
  • Assists with patient transfer or discharge, ensuring belongings are accounted for and communicating special needs to the receiving unit.
  • Maintains a clean, safe unit environment.
  • Responds to call lights and communicates patient requests to RN.
  • Demonstrates ability to respond appropriately to emergency situations.
  • Uses proper body mechanics.
  • Removes discontinued equipment to soiled utility room.
  • Reports unsafe/malfunctioning equipment and removes from use.
  • Cleans and disinfects equipment and returns to proper storage.
  • Restocks patient rooms with appropriate equipment and supplies.
  • Practices within infection control policies and procedures.
  • Takes and records vital signs, weight, height, intake and output (I/O) and notifies RN with abnormal results.
  • Provides appropriate skin care and reports areas of pressure, redness or abrasions.
  • Documents patient information on appropriate forms.
  • Acts as patient/family advocate by providing comfort, assistance, and information.
  • Communicates patient/family concerns and issues to appropriate nursing staff.
  • Provides report to following assistant at end of shift.
  • Recognizes and adopts activities appropriate to the specific need of the patient population.
  • Performs all work in accordance with safety procedures and requirements.
  • Participates in efforts to monitor and improve departmental CQI indicators.
  • Submits ideas for improving quality in the department.
  • Schedules self to complete requisite hours of training per quarter.
  • Develops a learning plan and tracks progress on a learning log.
  • Attends all department/hospital in-service orientation and other meetings as required.
  • Submits Dynamite ideas to leader.
  • Identifies improvement ideas within scope of responsibility and assumes accountability for implementation of plans and results.
  • Participates in hospital wide or departmental DIG each year.
  • Implements Just-Do-It initiatives in the department.
  • Interacts with co-workers in a team atmosphere.
  • Participates in hospitalwide TNT-related activities.
  • Participates in departmental recognition and socialization activities.
  • Attends all departmental staff meetings.
  • Establishes and maintains effective working relations with all associates.
