About The Position

The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. This role involves a range of direct resident care activities, documentation, and adherence to facility policies and procedures. The position requires maintaining a safe and comfortable environment for residents and collaborating with nursing staff.

Requirements

  • Must possess, as a minimum, an 10th grade education.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from managers and employees.
  • Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
  • CPR certification is required.

Responsibilities

  • Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
  • Identify residents before administering treatments or serving meals using wristbands or photo cards.
  • Record all entries on flow sheets, notes, and charts in an informative and descriptive manner using authorized abbreviations.
  • Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
  • Report all observed accidents and incidents as they occur.
  • Maintain resident confidentiality and report any suspected or known violations of protected health information disclosure.
  • Report any known or suspected unauthorized attempts to access the facility’s information system.
  • Perform only trained nursing care procedures.
  • Ensure resident rooms are ready for new admissions, including making beds and preparing admission kits.
  • Greet residents, escort them to their rooms, and introduce them to roommates and staff.
  • Make residents comfortable, inventory and mark personal possessions, and store clothing.
  • Assist residents with packing for transfers or discharges.
  • Transport residents to new rooms or receiving areas.
  • Assist with loading/unloading residents to/from vehicles.
  • Create and maintain a warm, personal, and positive atmosphere, and a calm environment.
  • Meet with nursing personnel to identify and correct problem areas and improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive nursing reports.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make beds and put extra covers on beds as requested.
  • Measure and record resident temperatures, pulses, and respirations (TPRs).
  • Weigh and measure residents as instructed.
  • Ensure residents who are unable to call for help are checked frequently.
  • Answer resident calls promptly.
  • Check residents routinely to ensure personal care needs are met.
  • Assist residents with identifying food arrangements, especially those with sight problems.
  • Record resident food/fluid intake and report changes in eating habits.
  • Keep resident water pitchers clean, filled, and within reach.
  • Perform after-meal care, including cleaning residents' hands, faces, and clothing.
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Follow work assignments and schedules.
  • Cooperate with inter-departmental and facility personnel.
  • Notify the facility when late or absent.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions such as dental/mouth care, bathing, hair combing, and dressing/undressing.
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist residents in preparing for medical tests.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Assist in preparing residents for physical examinations.
  • Check residents routinely to ensure personal care needs are met according to their wishes.
  • Observe and report skin breakdowns and pressure areas to prevent decubitus ulcers.
  • Report injuries of unknown source, including skin tears.
  • Observe disoriented and comatose residents, recording and reporting data.
  • Perform special treatments as instructed.
  • Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
  • Use only trained equipment and report defective equipment.
  • Maintain confidentiality of resident care information and report unauthorized disclosures.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to the supervisor.
