About The Position

Navitas Healthcare, LLC is seeking a dedicated and experienced CNA / Nurse Assistant to join an Acute Care Float team within a Hospital at Home program. In this role, you will deliver high-quality patient care in a dynamic acute care environment while supporting clinical teams and maintaining patient safety and comfort.

Requirements

• Minimum 1 year of acute care experience OR Certified Long-Term Care Nurse Assistant
• Previous nurse aide experience in a hospital setting or completion of an accredited Nurse Aide training program OR currently enrolled in a BSN program (junior level or completion of at least one clinical practicum)
• BLS Certification (required)

Nice To Haves

• High School Diploma or GED
• Experience working with diverse patient populations
• CNA Certification (preferred, where applicable)

Responsibilities

• Provide direct patient care under the supervision of licensed nursing staff
• Assist with daily living activities, including hygiene, mobility, and feeding
• Monitor and record vital signs and patient status
• Support care coordination within the Hospital at Home model
• Maintain accurate documentation and adhere to care protocols
• Ensure a clean, safe, and organized patient environment
