About The Position

To provide quality patient care by assisting the providers, RN, or LPN. Performs other duties as assigned. This role involves assisting with patient care activities, obtaining vital signs, maintaining a safe environment, and contributing to performance improvement initiatives. The position requires the ability to handle various patient populations and work in a potentially hazardous environment.

Requirements

  • High school education required.
  • CNA license have completed first semester of nursing school program or have an unlicensed assistive personnel equivalency required.
  • BLS required within 90 days of hire.
  • NVCI or equivalent class as determined to be required by the facility.
  • Medical terminology and normal patient vital signs.
  • Organization, problem-solving, computer skills, customer service, written and verbal communication skills.

Responsibilities

  • Assists in implementation of plan of care for all assigned patients daily.
  • Assists with activities of daily living throughout shift.
  • Assists patients with activity order as prescribed by provider each shift.
  • Obtains vital signs/weights on assigned patients as ordered daily.
  • Reports any abnormal findings to team leader immediately.
  • Assists staff nurses and/or providers on each shift.
  • Ensures patients have fresh water throughout shift, if provider orders allow.
  • Ensures each patient has an identification bracelet upon admission to the unit and upon bands being removed.
  • Updates white boards each shift.
  • Utilizing the ‘AIDET’ (Acknowledge, Introduction, Duration, Explain, Thank) model, conducts an appropriate introduction, reviews the plan of care, mutually sets attainable patient goals.
  • Bedside report and patient rounding to occur every shift between oncoming and off-going staff.
  • Attends unit staff meetings.
  • Maintains a safe and comfortable unit.
  • Keeps patient rooms and hallway free of clutter daily.
  • Ensures alarms on any assigned areas are audible to alert staff, each shift.
  • Removes malfunctioning equipment from the direct patient care area immediately upon discovering a problem.
  • Completes a requisition for repair and sends electronic work request before the end of the shift, pass on any issues to the next shift for follow up and provides a copy to the manager before the end of shift.
  • Utilizes correct patient identification by the hospital approved, two methods, for positive patient identification.
  • Acts rapidly and calmly during emergencies.
  • Demonstrates accountability to the nursing process through documentation.
  • Utilizes only approved abbreviations in patient care documentation each shift.
  • Documentation follows hospital and departmental policies for frequency and content.
  • Develops and promotes performance improvement initiatives.
  • Plans, develops and evaluates self-objectives, annually.
  • Participates in planning ways to solve problems and make improvements in the Medical Surgical unit, quarterly, and as needed.
  • Participates in implementing planned changes and activities to improve the Medical Surgical unit quarterly, and as needed.
