Requirements
- Active CNA certification
- Minimum 1 year LTC, rehab, or skilled nursing experience
- Ability to perform patient handling and mobility tasks
- Knowledge of basic patient care and vital signs
- Ability to float between units as needed
Responsibilities
- Provide direct patient care based on individualized care plans
- Assist with ADLs (bathing, grooming, hygiene, feeding, mobility)
- Perform patient transfers, repositioning, and ambulation support
- Measure and record vital signs, intake/output, weight, and outputs
- Provide hygiene care including bed baths, oral care, and skin care
- Support use of medical equipment (Hoyer lifts, wheelchairs, beds)
- Monitor and report changes in patient condition to nursing staff
- Maintain patient safety, infection control, and documentation
