About The Position

Rainbow Community Care is highly regarded for excellence in hospice, palliative, and guided care and invites you to join in our mission of enhancing lives throughout aging, illness, and loss. Rainbow Community Care is currently accepting applications for a dedicated Full Time CNA to work at the Inpatient Center in Johnson Creek. This person will work 11am to 8pm four days per week. Days will alternate based on week.

Requirements

Valid Wisconsin driver’s license and proof of current car insurance are required upon hire and must be maintained throughout employment.
Must be able to drive to and from other office/meeting locations as necessary.
Current Wisconsin Nursing Assistant Certification is required.
Must be able to maintain healthy boundaries and positive interpersonal relationships.
Must be tactful and courteous in communication with others, whether in-person, by phone, or in writing.
Must act with integrity and demonstrate good work ethics including reliable attendance and punctuality.
Ability to be self-directed and handle stressful and emergency situations.
Ability to maintain patient, family, and staff confidentiality.

Nice To Haves

Minimum of 6 months CNA experience preferred.
Hospice experience preferred.

Responsibilities

Assists with ambulation, transferring, and changing position of patients.
Assists the patient with personal grooming, toileting, incontinence care, bathing, skin care, nail care, dressing, transferring and walking.
Assists with preparing meals and assists patients with eating and drinking.
Gives emotional and psychological support to the patient/family.
Maintains a clean, safe and comfortable environment.
Performs light housekeeping duties as assigned.
Reads, understands, and follows the plan of care.
Maintains up-to date patient records and documents cares provided to the patient.
Reports changes in patient condition according to Rainbow policies.
