About The Position

Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The CNA Hospice Aide provides compassionate, hands-on care to patients and support to their families during end-of-life care. The role assists patients with daily living activities while promoting comfort, dignity, and emotional well-being. Working with the hospice care team, the aide provides respectful, individualized care in alignment with the patient's care plan. The patient care territory is Polk County. This is a per diem position.

Requirements

Current state certification or registration as a nurse assistant (CNA), in accordance with state requirements.
Current CPR certification.
Valid driver's license, reliable transportation, and liability insurance.

Nice To Haves

CNA experience within the last two years.

Responsibilities

Provide personal care services to patients.
Use safety principles and proper body mechanics when assisting with ambulation, transfers, range of motion, positioning, and household chores.
Plan and prepare nutritious meals and assist with feeding, as assigned.
Perform homemaking and other environmental services, as assigned.
Observe, report, and document changes in a patient's status; report to the primary nurse if unable to perform a task.
Follow specific written personal-care instructions prepared by a registered nurse in accordance with the plan of care; receive the instructions before delivering services.
Assist patients with prescribed exercises as instructed by a nurse or therapist.
Perform and record simple urine tests for sugar, acetone, and albumin.
Perform other duties as assigned.
