About The Position

The CNA Coordinator is responsible for the day-to-day operations of the hospice home health aide staff. This position will be responsible for hiring, training, retention, and scheduling of the home health aides in conjunction with the demands of the hospice program. The CNA Coordinator is responsible for ensuring that the annual supervisory requirements for the home health aides have been met and competencies are completed and accurate. This position also manages the supply inventory with oversight from the Executive Director (or designee).

Requirements

Must have the strength and endurance required for standing, walking, lifting, bending, pushing & pulling on a consistent basis up to 50 lbs.; must be able to go up and down stairs carrying up to 20 lbs.
Must be able to move intermittently throughout the workday
Must have the strength and endurance required for sitting for lengthy periods of time
Must possess sight/hearing and speech senses, or use prosthetics that will enable these senses to function adequately so that the requirements of this position are fully met
Category 2 for potential exposure to blood/body fluids. (Does not usually require the performance of procedures or other tasks in the work routine that involve exposure to blood, body fluids or tissues, but Category 2 tasks may require the unexpected performance of these procedures.)
Must be able to handle frequent interruptions and idiosyncrasies of people
Must be able to comprehend English and follow oral and written instructions
Must utilize appropriate PPE as needed
Must be able to follow the designated plan of action in the event of a fire or other emergency
Must use appropriate body mechanics as needed
While performing the duties of this job, the employee needs to be able to handle work- related stresses
The noise level in the work environment is usually minimal
Licensed as a Certified Nurse Assistant, Licensed Practical Nurse, or Registered Nurse by the State Board of Nursing in the State of Nevada (preferred)
Must be able to read, speak, write, and understand the English language
Must be able to cope with emotionally charged situations encompassing patients, families and other caregivers
Must be available for pre-scheduled, on-call hours, weekends, and holiday as needed
Must have reliable transportation, a valid driver’s license and maintain current auto insurance
Must have CPR/BLS Certification

Nice To Haves

Prefer 1 year experience as a Certified Nurse Assistant, Licensed Practical Nurse or Registered Nurse in hospice, home health or geriatric care

Responsibilities

Assists with hiring, training, and retention of CNA staff to meet the demands of the hospice agency
Ensures that new CNAs have and maintain proper credentials per state, federal and CHAP requirements
Meets all competency requirements as required by state, federal and CHAP guidelines
Schedules CNAs according to the individualized Plan of Care established by the RN Case Manager for each patient
Serves as the point of contact for patients and/or caregivers for CNA scheduling needs, concerns and grievances
Is responsible for the coordinating, ordering, and maintaining records of the ProCare Hospice supply needs
Immediately reports any changes of staff to the RN Case Manager and Director of Nursing or designee
Coordinates delivery of materials or supplies with patient supply companies to a schedule within business hours that is not disruptive to IDT meetings or other training
Educates CNA staff on the use and potential hazards of patient care supplies, when necessary
Generates miscellaneous patient reports when needed
Participates in Performance Improvement Activities as requested
Participates in QAPI Compliance goals for CNA documentation
Completes appropriate documentation in a legible and timely manner
Ensures CNA compliance with attending staff meetings and in-service training
Stays current with all mandatory agency and regulatory requirements
Adheres to patient rights, abuse reporting and confidentiality policies
Attends scheduled staff meetings and or in-service trainings
Adheres to the departmental dress code requirements related to personal grooming and attire
Demonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness, participating in required meetings
Demonstrates ability to problem-solve and make decisions by assessing situations and considering alternatives, seeking guidance and direction as necessary, utilizing proper chain of command
Demonstrates effective time management skills by coordinating activities to achieve maximum productivity and efficiency, willingness to adjust or revise work schedules as requested
Assumes responsibility for on-going personal development and continuing education
Pursues professional networking opportunities to stay current in the fields of social work and hospice
Completes assigned projects within specified time frames
Assists with supervision and guidance of agency social work assistants and student interns as directed
