About The Position

Under the direction and supervision of a licensed nurse, performs basic nursing care procedures. This role involves assisting residents/patients with daily living activities, monitoring their condition, and documenting care. The position requires adherence to facility policies, infection control practices, and participation in educational programs.

Requirements

  • State of Connecticut CNA Certification; or within 120 days of CNA Certification course completion.
  • Ability to interact professionally with coworkers.
  • Prioritize tasks.
  • Function as a team member.
  • Ability to speak, read English in order to communicate with patients, residents and clinical staff.
  • Provides care and services that demonstrate behaviors and the principles of person-centered care including affording people dignity, respect and compassion, offering coordinated care, support and treatment and care that is personalized.

Nice To Haves

  • High School diploma or GED preferred
  • 6 months – 1 year experience as a CNA preferred

Responsibilities

  • Performs basic nursing care procedures under the direction and supervision of a licensed nurse, including assisting with meals, transfers, ambulation, bathing, toileting, dressing, and grooming.
  • Observes and communicates pertinent information about the resident/patient’s condition to appropriate facility staff.
  • Takes and records pulse, respirations, blood pressures, and weight as needed.
  • Documents care and observations on worksheets and EMR accurately and in a timely manner.
  • Maintains cooperative relationships to enhance the efficient operation of the care team by providing support and assistance to co-workers.
  • Adheres to infection control practices.
  • Supports the psychosocial needs of the residents as per the plan of care and profile.
  • Participates in educational and inservice meetings to update knowledge of techniques, guidelines, and regulations, including mandatory programs and continuing education.
  • Uses equipment in an appropriate and safe manner, adhering to standards of the Diligent Minimal Lift Program.
  • Follows facility policies and procedures.
  • Performs other duties as required.
