About The Position

Provides resident care as assigned and per individual resident routine, according to standards of care, under CMS Federal and State Regulations for Skilled Nursing Facilities, and facility policies and procedures. This role involves detailed documentation of resident cognitive patterns, mood, and behavior, identifying and reporting changes in functioning, and assisting residents with daily living activities. The CNA is responsible for grooming residents according to their preferences, understanding the care provided by therapists, and individualizing care based on resident needs and preferences. They must also identify and document how residents communicate their needs and respond to call lights promptly. The position requires maintaining a clean work environment, managing supplies, floating to various units as needed, participating in quality improvement activities, and providing training to new CNAs.

Requirements

  • One year experience as a CNA
  • Continued education that includes: Communication and Interpersonal Skills, Safety and Emergency Procedures, including Heimlich Maneuver, SNF based CNA skills, Mental Health and Social Services of Residents, Personal Care Skills, Care of Cognitively Impaired Residents, Basic Restorative Services, Promoting Resident’s Independence and Respecting Resident’s Rights, Abuse and Neglect Training, Facility Infection Prevention, Dementia Training
  • Current CNA Certification in state of practice
  • Approved CPR or BLS certification required within 90 days

Responsibilities

  • Provides resident care as assigned and per individual resident routine, according to standards of care, under CMS Federal and State Regulations for Skilled Nursing Facilities, and facility policies and procedures.
  • Identifies and documents in the medical record the resident's usual and current cognitive patterns, mood and behavior, and whether these present a risk to the resident or others.
  • Identifies and documents how the resident typically displays personal distress such as anxiety or fatigue.
  • Recognizes a change in the resident and the resident's functioning and describes and reports such changes to the nurse.
  • Performs and documents daily range of motion exercises.
  • Assists the resident to transfer from bed to wheelchair.
  • Assists residents daily with bathing, dressing, grooming, transfers and ambulating through the facility, toileting, and eating.
  • Grooms residents as they wish to be groomed.
  • Describes the care, services and expected outcomes of care they provide.
  • Identifies and documents, to the extent possible, factors that may underlie the resident's expressions of distress.
  • Applies knowledge of lifelong patterns, preferences, and interests for daily activities to enhance quality of life and individualize routine care.
  • Identifies and understands that the resident with dementia may be responding predictably given the situation or surrounding.
  • Matches activities for a resident with dementia to his/her individual cognitive and other abilities and tracks the specific behaviors in that resident based on the assessment.
  • Identifies and documents how the resident communicates physical needs such as pain, discomfort, hunger or thirst, as well as emotional and psychological needs such as frustration or boredom; or a desire to do or express something that he/she cannot articulate.
  • Answers call lights in a timely and appropriate manner ensuring that all of the residents needs to be provided by C.N.A. are met.
  • Documents all daily care provided according to Federal and State Regulations and standards of care.
  • Maintains a clean work and patient environment.
  • Maintains supplies, including scanning, stocking and ordering new supplies and disposing of expired or damaged supplies.
  • Floats to various units based on identified needs.
  • Participates in department Quality Assurance and Performance Improvement activities.
  • Provide leadership and training to new CNA's.
