About The Position

This is a limited-term role (6-8 months) with the possibility of extension, focused on serving members and contributing to an award-winning culture. The role supports the vision, quality standards, and practice development of Interdisciplinary Team (IDT) staff within the Care Management Division. The Best Practice Leads guide and support care management practice by serving as a resource to staff in areas such as care planning, coordination of care, and conducting onboarding and training. This role is responsible for assisting in the development and deployment of policy, procedure, processes, and guidelines specific to care management practice, adhering to DHS Family Care Contract, DHS directives, and technology changes. The CM BPL Education Specialist role emphasizes comprehensive, engaging, and effective adult learning techniques and content deployment. This position is the primary CM BPL team member responsible for creating and managing access to IDT staff training modules within Lakeland Care Inc.’s (LCI) learning management system.

Requirements

  • A four-year bachelor’s degree in adult education with one (1) year experience working with Family Care population.
  • A four-year bachelor’s degree in human services or related field or be a Certified Social Worker in the State of Wisconsin with one (1) year experience working with Family Care population.
  • A four-year bachelor’s degree in any other area than human services with three (3) years’ experience working with Family Care population.
  • A current RN License to practice in the State of Wisconsin, preferably with two (2) of skilled nursing experience in gerontology/disabilities and/or home care.
  • Experience in Adult Learning/Education and training in design, development, and delivery.
  • Excellent computer, communication (verbal and written), and presentation skills.
  • Ability to plan, prioritize, and organize work effectively to meet assigned timelines.
  • Leadership qualities necessary to support, mentor, and train staff and peers.
  • Ability to work from various work environments throughout the LCI service regions and state of Wisconsin as needed.
  • Ability to access members’ homes which are not required to comply with the ADA regulations.
  • Ability to lift up to 25 pounds.
  • Current driver’s license, acceptable driving record, and proof of adequate insurance required.

Responsibilities

  • Manage and analyze data to identify trends, gaps, and root causes, presenting findings with recommended solutions.
  • Assist in the development, deployment, and maintenance of content and materials for reinforcement/refresher trainings and onboarding.
  • Assist in deployment of policy/procedure, processes, guidelines, and best practices.
  • Leverage advanced tools and system capabilities, including AI, to streamline data analysis, enhance workflows, improve efficiency, and drive insight-informed decisions.
  • Work with Program Operations departments to enhance consistency in formatting resources, language, and communications.
  • Research, design, and implement adult learning strategies rooted in best practices, focusing on engagement, real-world application, and sustained knowledge retention.
  • Evaluate, monitor, and respond to practice trends to promote high-quality, effective care management.
  • Advance organizational practices and processes to align with larger strategic efforts.
  • Support care management by leading IDT staff onboarding, electronic member record testing and application projects, and performing ‘fill-in’ tasks.
  • Assist with care management-related activities and projects to support organizational needs or goals.
  • Provide support to CM Supervisors and IDT staff in accordance with LCI policy regarding the care planning process.
  • Partner with other LCI departments in developing and reviewing resources, data, and trends for Care Management.
  • Support mentoring, acclimating, and onboarding new care management employees.
  • Support new lines of LCI business requiring CM resources.
  • Participate in and support LCI’s evolution of the Electronic Medical Record system, including testing and deployment of system changes and upgrades.
  • Represent LCI to the community through professional interaction, clinical consultation, public speaking, media presentations, and participation in community advisory groups as requested.
