This position is located at the SC Department of Behavioral Health and Developmental Disabilities (DBHDD) - Office of Mental Health, Transitions-Orangeburg, 2319 St. Matthews Rd. Orangeburg, SC 29118. As the Client Advocate II, you will effectively coordinate Community Long Term Services to assigned participants in accordance with SCDHHS/CLTC area offices.

Requirements

  • Bachelor or graduate degree from an accredited college or university in a health or human services field that promote the physical, psychosocial and/or vocational wellbeing of the individual being serviced and documentation of at least two years of experience working with target population.
  • The degree must be from an institution that is accredited by a nationally recognized educational accrediting body.

Nice To Haves

  • Bilingual abilities in English and Spanish (or another language) are a plus.
  • Ability to coordinate interaction between staff, document experience, skills or training with participation to develop goals.
  • Possess knowledge of community resources and a working knowledge of families and/or systems theory.

Responsibilities

  • Provide CLTC case management services to CLTC participants.
  • Provide to face to face contact with participant during initial, quarterly and annual visits.
  • The case manager will also follow up with participate on a monthly basis.
  • Develop and work collaboratively with other providers, public and private, and division with SCOMH/SCDHHS to provide clients with the most appropriate and effective care.
  • Comply with federal, state, and third-party payor regulations in the delivery of covered services ensuring these are appropriate and that documentation requirements are met as indicated by the SCOMH, SCMOMH Corporate Compliment, and SCDHHS.
