About The Position

Intensive HomeBase Family Services - Clinician. Help children and families create meaningful, lasting change—right in their homes and communities. Bay State Community Services (BSCS) is seeking a compassionate and skilled Clinician to join our Intensive Home Based Family Services team. This is an exciting opportunity for a clinician who enjoys working collaboratively with children, youth, and families and wants to provide services where they can have a meaningful impact. Our clinicians work as part of a supportive multidisciplinary team while providing intensive, strengths-based and trauma-informed services in home and community settings.

Requirements

  • Master’s degree that is license-eligible with an Allied Mental Health licensure board of the Department of Public Health with appropriate scope
  • In not, license eligible a master degree in one of the following fields with 2 years of experience working with children and families with behavioral health needs: Mental Health (including but not restricted to counseling, addictions, family therapy, social work, psychology, and expressive therapies), child development, nursing/medicine and Education with a concentration in Behavioral Health
  • Must demonstrate sound knowledge and understanding of clinical theory and practice, inclusive of: Strength Based Services Healthy Child Development Family Systems Behavior Management Behavioral Health Risk Assessment and Safety Planning
  • Must possess strong organizational and clinical communication skills
  • Must be available for rotation in in-person on-call, when required
  • Must have flexibility in job schedule to meet families’ needs; ability to work some evenings/weekends
  • Computer proficiency
  • Valid MA driver’s license acceptable driving record, reliable transportation and proof of insurance
  • Acceptable background record check as required by program

Responsibilities

  • Work in a strength-based family focused team approach
  • Provide intensive family therapy and robust care coordination/targeted case management in-person in the home and community
  • Strengthening family systems so that the youth can be supported in her/his/their home and community to prevent hospitalization and higher level of care
  • Complete initial intake, comprehensive assessment and include the caregiver and youth in the development of a treatment plan
  • Assess safety needs and develop a safely plan at the onset of care and ongoing
  • Completes all documentation in a timely manner, including assessment, treatment/individualized action plan, progress notes and discharge summary; accurate observation and documentation of the progress, behavior, issues, and performance of persons being served; note any report changes in physical or emotional health of the persons served
  • Exhibit professionalism in all job functions (confidentiality, boundaries, respect for other employees), appropriate representation of the agency at all times (public, families and vendors)
  • Participate actively and positively in staff meetings, supervision, professional development and annual performance evaluations, both internal and external
  • Attend mandatory trainings
  • Maintain necessary caseload to meet consistent productivity expectations
  • The clinician and team will meet with the family 3-5 times per week or more intensely as to prevent the need for admission to a 24-hour level of care
  • Will work collaboratively with other team members, including formal and natural supports in addition to support community referrals and linkages
  • Provide transportation to youth and families
