About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct flexible, comprehensive, and personalized health evaluations at patients' homes. The visit, lasting approximately 45 minutes, includes a medication and medical history review, a physical evaluation, and potentially ordered diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The role requires flexibility to travel locally and within licensed states, and may involve conducting virtual visits. The clinician will provide all Signify Health services, including Diagnostic & Preventive Services products like spirometry, DEE, and bone density screening. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and airborne allergens.
