About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct approximately 45-minute visits, including medication and medical history review, physical evaluation, and potentially diagnostic tests. The role utilizes an iPad with an intuitive clinical workflow and does not require prescribing medication, ordering lab tests, or altering current treatment regimens. The primary workplace is the private residence of health plan members, which can be diverse and may contain household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these environments.

Responsibilities

Provide In-Home Health Evaluations.

Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
Be flexible to travel locally and within licensed states, as needed.
