About The Position

Signify Health, part of CVS Health®, is seeking a Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in the Monterey California area. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The position offers flexibility to travel locally and within licensed states, and may include virtual visits and providing various Signify Health services such as Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).

Requirements

Active, unrestricted license(s) in coverage area(s).
Board certification.
Able to cover the following primary locations: San Jose including Monterey County area.

Responsibilities

Conduct In-Home Health Evaluations.

Conduct diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).

Travel locally and within licensed states, as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products.
Conduct virtual visits as needed.
