In-Home Health - NP or PA (Part Time) - Olympia, WA - Grays Harbor
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and other airborne allergens or irritants. Candidates must be able to perform duties comfortably and safely in these environments.
Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)

Responsibilities
- Conduct In-Home Health Evaluations.
- Engage with people in the comfort of their own homes.

- Bring flexible, comprehensive and personalized health evaluations right to people’s front doors.
- Connect one-on-one with individuals and answer health questions.
- Conduct medication and medical history reviews.
- Perform physical evaluations.
- Conduct additional diagnostic tests if ordered by the person’s health plan (i.e., diabetic eye exams, spirometry, etc.).
- Use an iPad and an intuitive clinical workflow for each evaluation.

- Be part of efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs.
- Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
- Be flexible to travel locally and within licensed states, as needed.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
