About The Position

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The clinician will conduct flexible, comprehensive, and personalized health evaluations at patients' residences. The evaluations, lasting approximately 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests if ordered by the health plan. Clinicians will use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The role is part of a national network of clinicians dedicated to improving health outcomes and connecting people to the right care.

Responsibilities

Provide In-Home Health Evaluations.

Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
