In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Chemung County, Elmira, NY
About The Position
Signify Health, part of CVS Health®, is seeking a Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations in the Chemung County, Elmira, NY area. This role involves engaging with people in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of members, which can vary in conditions and may contain household pets, secondhand smoke, or other airborne allergens.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification.

Responsibilities
- Conduct in-home health evaluations.
- Review medication and medical history.
- Perform physical evaluations.
- Conduct diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry).
- Use an iPad and an intuitive clinical workflow.
- Travel locally and within licensed states as needed.
- Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products.
- Conduct virtual visits as needed.
