In-Home Health - Physician (Part Time) - Kings County, NY
About The Position
Signify Health, part of CVS Health®, is seeking a Part-time Physician to provide In-Home Health Evaluations. This role involves engaging with patients in their homes to bridge gaps in care and build trusted relationships. The evaluations, lasting about 45 minutes, include a medication and medical history review, a physical evaluation, and potentially diagnostic tests. Physicians will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions. Candidates must be comfortable working in environments that may contain household pets, secondhand smoke, and other airborne allergens or irritants.
Requirements
- Active, unrestricted license(s) in coverage area(s).
- MDs and DOs.

Responsibilities
- Conduct In-Home Health Evaluations.

- Bring flexible, comprehensive and personalized health evaluations right to people’s front doors.
- Conduct virtual visits as needed.
- Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.).
- Travel locally and within licensed states, as needed.
