About The Position

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with individuals in their homes to bridge gaps in care and build trusted relationships to improve health outcomes. The evaluations, lasting about 45 minutes, include medication and medical history review, a physical evaluation, and potentially ordered diagnostic tests. Clinicians use an iPad with an intuitive workflow and are not required to prescribe medication, order lab tests, or alter current treatment regimens. The primary workplace is the private residence of health plan members, which can vary in conditions and may include household pets, secondhand smoke, and airborne allergens.

Responsibilities

Provide In-Home Health Evaluations.
Engage with people in the comfort of their own homes.
Bridge gaps in care.
Build trusted relationships to make people healthier.
Bring flexible, comprehensive and personalized health evaluations right to people’s front doors.
Conduct one-on-one visits to answer health questions.
Perform medication and medical history reviews.
Conduct physical evaluations.
Perform additional diagnostic tests if ordered by the person’s health plan (e.g., diabetic eye exams, spirometry).
Use an iPad and an intuitive clinical workflow for each evaluation.
Join a national network of purpose-driven clinicians dedicated to improving health outcomes.
Participate in efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs.
Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role.
Be flexible to travel locally and within licensed states, as needed.
Conduct virtual visits as needed.
Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.).
