About The Position

This is a hybrid role requiring some office hours and some virtual telehealth hours. The position is 32 hours per week, Monday through Thursday from 12:00 PM to 8:30 PM. The role requires independent work, and experience with group work and Intensive Outpatient Programs is a plus. The hourly rate varies based on licensure and experience, with additional pay for bilingual services. Pay rates for union positions are set in the Collective Bargaining Agreement with SEIU Local 521, with six steps from entry-level to 15 years of employment.

Requirements

  • Master's degree in the mental health profession and one year's experience working in human services settings.
  • Eligibility for waiver and apply for registration and waiver upon hire or as soon as regulations allow and obtain waiver within six months of hire date.
  • Eligibility for registration from the California Board of Behavioral Sciences (BBS) as an Associate Marriage, Family Therapist (AMFT), Associate Clinical Social Worker (ACSW), or Associate Professional Clinical Counselor (APCC), OR eligibility for registration from the California Board of Psychology (BOP) as a Registered Psychologist or a Psychological Assistant.
  • First Aid Certification or successful completion of First Aid Certification within 30 days of hire, if assigned to residential facilities.
  • Meet government and/or regulatory standards for continuing education credits and recertification, including a minimum of 20 hours of directly related professional training, if assigned to a residential facility.
  • Proven typing speed of 30 net wpm.
  • Computer literacy; ability to use MS Office applications—Word; Excel; Outlook—and Windows-based databases at a basic level.
  • Demonstrated ability to work independently and in a small team environment.
  • Demonstrated ability to organize and manage a caseload; manage multiple tasks and demands on a timely basis.
  • Demonstrated ability to engage and communicate effectively with others in both verbal and written English; legible handwriting.
  • Demonstrated ability to conceptualize and prioritize the problems of mentally disordered individuals, determining their needs, services, and appropriate treatment methods.
  • Demonstrated ability to monitor and support developmental growth and the recovery process.
  • Demonstrated ability to effectively conduct and facilitate group rehabilitation services.
  • Demonstrated ability to maintain timely, accurate and complete medical charts and client records and prepare reports.

  • Demonstrated ability to work from a strengths-based, harm-reduction, social rehabilitation model.

  • Demonstrated ability to work collaboratively with individuals identified as those with serious mental illness who meet specific criteria.
  • Demonstrated ability to exhibit exemplary customer service, compassion, and care in the execution of all duties and interactions.
  • Must be able to meet and receive a criminal records clearance, as required by Title XXII, other licensing regulations, and Momentum practices.
  • Physical requirements needed to perform the essential functions of this job, with or without accommodation: Hearing and talking on telephone and in person; standing, walking, sitting, pinching and finger flexion are required constantly (over 2/3 of the workday); Lifting, carrying, pushing, pulling, bending, stooping, crouching, and kneeling are required occasionally (under 1/3 of the workday); Must be physically able to assist clients in the evacuation of program facilities in emergency situations.

  • Demonstrated ability to work with participants who may have patterns of service that relied almost exclusively on emergency and institutional care.
  • Demonstrated ability to work with participants who may have cognitive difficulties and medical co-morbidities and are considered medically fragile and may also be developmentally disabled.
  • Demonstrated ability to work with participants who may have long-term patterns of homelessness and criminal justice involvement.
  • Knowledge of Clinical techniques for assessment, treatment, and rehabilitation of people with mental illness; problem resolution and conflict resolution techniques.
  • Knowledge of Community resources, including those required to meet the non-mental health needs of the target population.
  • Knowledge of Mental Health Services Act (MHSA) and Wellness Recovery Action Plan (WRAP).
  • A valid California Driver’s License, access to a reliable vehicle, proof of auto liability insurance, and a clean DMV record are preferred for La Selva Outpatient programs.

Responsibilities

  • Provide clinical leadership in comprehensive assessments, case conferences, treatment plans, and strategic interventions.
  • Serve as a lead member of the treatment team, participating in crisis intervention, clinical supervision, and direct services.
  • Provide orientation, consultation, and coordination within the work team to ensure service delivery meets client, family, and payor needs.
  • Serve as shift leader for a particular shift, leading the team and program.
  • Provide clinical leadership, training, and guidance to staff, interns, and volunteers.
  • Provide consultation and backup to support other staff in responding to client crises.
  • Develop, review, evaluate, and sign documentation, including those requiring LPHA signature.
  • Collaborate with Benefits Specialists and Intake Specialists and engage insurance providers to obtain authorization for treatment.
  • Provide treatment services, training, and support to clients individually and in groups, and associated services to clients’ families and/or representatives.
  • Interview clients and representatives to gather demographic and psychosocial information.
  • Complete strengths assessments and needs analyses with clients and families.
  • Develop goal-oriented, time-limited, comprehensive plans in cooperation with clients and/or representatives.
  • Facilitate or lead groups through rehabilitation counseling processes to develop and strengthen life, coping, and independent living skills; develop curriculums.
  • Assist clients with understanding their problems and developing plans to resolve them.
  • Maintain regular communication with families and authorized third parties involved in the rehabilitation process.
  • Coordinate needed services for clients, their families, and others significant to service plans.
  • Make referrals to other public and private social services and community agencies and resources.
  • Develop recommendations for discharge and follow-up treatment planning for clients and their families.
  • Complete comprehensive reassessments prior to expiration of current treatment plans.
  • Maintain timely, accurate, and complete clinical records and other required documentation and produce clear, accurate reports.
  • Provide assessment and intervention in response to client crisis, including necessary follow-up and using appropriate consultation; provide coverage in emergency situations.
  • Assist Program Manager in maintaining compliance with CARF, contract, Agency, and regulatory requirements.
  • Provide transportation in personal or agency vehicle, including transportation of clients and/or program supplies for agency business.
  • Provide on-call coverage and participate in after-hours rotation, as assigned.
  • Perform First Aid in emergency situations, if assigned to residential facilities.
  • Attend and participate in staff, program, agency, and community meetings and training, as required.
  • Meet continuing education requirements to satisfy regulatory, accreditation, and Agency requirements.
  • Participate in management of team/program discretionary resources available to meet client needs.
