About The Position

Bay Cove Human Services' mission is to partner with people to overcome challenges and realize personal potential. Bay Cove pursues this mission by providing individualized and compassionate services to people facing the challenges associated with developmental disabilities, mental illness, substance use disorders, and homelessness at more than 175 program sites throughout Greater Boston and Southeastern Massachusetts. The Social Worker, or Mental Health Clinician is responsible for managing multiple clients and their individual needs. Their duties include meeting with clients to discuss recent successes or setbacks to their situations, defining the clients vision, and developing treatment plans for clients with Intellectual and Developmental Disabilities, as well as Mental Health and/or Substance Abuse issues or recovering from traumatic events. The Clinician is frequently communicating with clients to ensure their physical and mental health. This role drives outreach and engagement, social needs assessment, DS care planning and care coordination, support of care transitions, health and wellness coaching, as well as community and social services connections in partnership with the individuals and their designated care teams.

Requirements

  • Masters Degree in Mental Health Counseling, Social Work, Family Therapy or related field.
  • Licensed or license eligible by Board or Professional Licensing.
  • Effective skills in managing, teaching, and negotiating, and in collaborating with multidisciplinary teams and client/family focus.
  • Experience working with people living with disabilities with complex LTSS and BH needs.

Nice To Haves

  • Minimum of 3 years of clinical experienced preferred.
  • Preference given to bi-lingual/bi-cultural applicants and those with lived experience.
  • In some cases, experience may be substituted for academic training.

Responsibilities

  • Providing mental health counseling to individuals, groups or families
  • Conducting initial assessments of clients' situation to determine needs and goals
  • Researching and advocating for appropriate public assistance resources for clients
  • Communicating with clients' care teams
  • Providing crisis intervention as needed
  • Referring individuals to appropriate treatment centers, as indicated
  • Ensuring that all case files, and other records, strictly comply with policies, regulations, and procedures
  • Coordinating treatment planning and maintaining ongoing contact with outpatient providers for the continual care of patients
  • Actively participating in ongoing training as needed in order to meet all certification standards and credentialing policies
  • Inform Individuals of options for specific services, programs, and providers that best meet identified needs
  • Perform other duties, as required
