Capital Health is seeking a Clinical Wound Care Nurse for its Inpatient unit. This role provides consultation and direct clinical care to patients requiring specialized management for skin, wound, and ostomy care, as well as incontinence management. The nurse will be responsible for documenting assessments, care plans, interventions, and outcomes in the electronic medical record. This position promotes best practices in patient care and nursing, assesses the need for patient education, and implements and documents educational interventions. The nurse will also educate staff nurses on various aspects of wound and skin care, identify discharge needs, and collaborate with Case Management for follow-up care. Consultation with a Wound Care specialist physician or Wound Care Center for post-discharge care may be recommended. The role involves analyzing wound patterns to identify areas for improvement and collaborating with the Clinical Education Department to develop competencies and educational modules for wound care. Input and feedback on wound and ostomy care policies and procedures are expected, along with maintaining and developing knowledge and skills in wound, ostomy, and continence therapy. Travel between two main campuses may be required. The position also includes performing other assigned duties.
