APRN - Wound Care - Per Diem
About The Position
The APRN - Wound Care position involves the debridement of wounds using various methods, application and management of topical wound care products, and directing associated services to improve wound healing outcomes. This includes interpreting diagnostic tests, recommending follow-up actions, and prescribing medications or Durable Medical Equipment (DME) within the scope of practice and collaborative agreement. The role also includes performing Diabetic Foot Ulcer risk assessments, educating patients and physicians on wound management, and providing updates on patient conditions. The APRN will act as a resource for decisions, planning, and policies affecting resident health, and collaborate with staff to identify and meet customer needs. Other duties may be assigned.
Requirements
- Registered nurse (RN) preparation and Nurse Practitioner (NP) license required.
- Minimum of 1-year experience as an NP.
- Minimum of 5 years’ experience as an RN.
Nice To Haves
- Certification from the Wound Ostomy and Continence Nursing Certification Board preferred.
Responsibilities
- Debridement of wounds by sharp, enzymatic, autolytic or surgical methods.
- Application and management of topical wound care and associated wound management products.
- Direction and recommendation of associated services to improve wound healing outcomes (labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, orthotics).
- Interpretation of diagnostic tests and recommendation of appropriate follow up.
- Recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME).
- Diabetic Foot Ulcer risk assessment for Members with Diabetes, as per the ADA guidelines.
- Education of patients as to the topical and systemic management of their wounds.
- Education of the Primary Care Physician (PCP), CM, or referring Physician on current wound management practices.
- Provision of regular updates to the PCP, CM, or referring Physician on the status of their patient’s systemic conditions associated with wound care.
- Acts as a resource person for decisions, planning, and policies affecting the health of residents.
- Collaborates with staff to identify customer needs and to help develop plans to meet these needs.
- Other duties as assigned.
