Job Overview Cedars-Sinai is currently looking for a Clinical Value Nurse Practitioner to join our dynamic team in Los Angeles, CA. As a Clinical Value Nurse Practitioner, you will be instrumental in implementing and managing value-based care programs that set the standard for excellence in healthcare.

Requirements

  • Graduate of an accredited Nurse Practitioner program approved by the California Board of Registered Nursing (ANCC)
  • Two (2) plus years of clinical and healthcare operations experience with demonstrated success leading value-based care, care management, population health, quality improvement, or clinical efficiency initiatives.
  • Strong experience collaborating with physicians and multidisciplinary teams to reduce length of stay, improve care transitions, reduce readmissions, and achieve quality and cost-performance goals.
  • Current CA RN License
  • Nurse Practitioner and Furnishing License
  • National Certification as a Nurse Practitioner
  • Current DEA License
  • Current Life Support Certification required

Responsibilities

  • Lead the implementation of departmental programs aimed at improving value-based care.
  • Develop and complete interventions to streamline inpatient care, reduce length of stay, and minimize costs while maintaining high-quality care.
  • Conduct regular updates and provide insights on the status of value-based programs to department leaders and staff.
  • Identify eligible patients and provide direct and indirect clinical care services, including diagnosis and treatment, under the supervision of the designated physician lead.
  • Perform daily rounding and collaborate with clinical attendings, nursing staff, pharmacy staff, and case managers.
  • Coordinate hospital discharge processes and connect patients to appropriate outpatient services.
  • Ensure efficient post-discharge care through education and patient compliance support.
  • Foster collaborative relationships with medical staff to promote value-based programs and optimize patient outcomes.
  • Track and analyze outcomes for patients in value-based programs, including utilization, cost, efficiency, and clinical outcomes.
  • Participate in the Clinical Value Consortium to address organizational barriers and share guidelines.
  • Promote value-based care principles across the organization and educate others on guidelines.
