About The Position

This position provides direct service to patients in a Behavioral Health Program area. The schedule is 36 hours per week, Monday-Friday, during day shifts at the Mercyhealth Behavioral Health Clinic in Crystal Lake, IL. The pay range is $30.59/hr - $53.41/hr, based on licensure level. The role involves completing psychosocial assessments and treatment plans, conducting comprehensive assessments of clinical needs, developing treatment plans with patients, delivering behavioral health care within the scope of licensure, providing crisis assessment and intervention, consulting and coordinating care with other professionals and agencies, providing discharge planning, referring patients to community resources, participating in clinical collaboration and supervision, maintaining patient data and documentation, attending required meetings, and participating in performance improvement activities. The position requires compliance with all Mercy Health System, State, and JCAHO policies and regulations.

Requirements

  • Master's degree in counseling, Social Work, Psychology or other related field
  • Must possess all required certification and licensure per the state in which they will be practicing.
  • Wisconsin: LCSW, LPC, or LMFT
  • Illinois: LCSW, LCPC, LMFT

Nice To Haves

  • TACT Level 2
  • CPR/BLS preferred

Responsibilities

  • Completes psychosocial assessments and treatment plans in cooperation with patient and multidisciplinary team.
  • Conducts a comprehensive assessment of the clinical needs of each patient prior to beginning individual, family or group therapy.
  • Works collaboratively with the patient to develop a treatment plan addressing patient’s identified treatment goals.
  • Delivers behavioral health care in accordance with patient evaluation, treatment plan and within the scope of their license.
  • Works directly in treatment relationship with patients, individually or in groups, in consultation with Psychiatrist and or NP.
  • Provides crisis assessment and intervention.
  • Consults and coordinates care of patient with physician and or NP, therapists, and community agencies.
  • Provides appropriate discharge planning.
  • Refers patient or patient's family to other community resources.
  • Participates in clinical collaboration and or psychiatry supervision to discuss cases and determine appropriate course of treatment.
  • Maintains pertinent patient data and documentation in medical record in accordance with state, TJC and Mercyhealth system guidelines.
  • Attends required meetings or staffing as indicated by Program Manager.
  • Participates in performance improvement activities.
  • Performs other duties as assigned.
  • Complies with all Mercy Health System, State and JCAHO policies and regulations.
