This position provides direct service to patients in a Behavioral Health Program area. The role involves completing psychosocial assessments and treatment plans in cooperation with the patient and a multidisciplinary team. A comprehensive assessment of the clinical needs of each patient is conducted prior to therapy. The therapist works collaboratively with the patient to develop a treatment plan addressing their identified goals and delivers behavioral health care in accordance with the patient's evaluation, treatment plan, and within the scope of their license. This includes working directly in a treatment relationship with patients, individually or in groups, in consultation with a Psychiatrist and/or NP. The role also involves providing crisis assessment and intervention, consulting and coordinating care with physicians, therapists, and community agencies, and providing appropriate discharge planning and referrals to community resources. Participation in clinical collaboration and/or psychiatry supervision is required to discuss cases and determine appropriate treatment courses. The position also requires maintaining pertinent patient data and documentation in the medical record according to state, TJC, and Mercyhealth system guidelines, attending required meetings, and participating in performance improvement activities.
