About The Position

Concierge Home Care is seeking a Clinical Team Support professional, specifically a Scheduling Coordinator for Home Health. This role is crucial in ensuring clinicians are scheduled efficiently and patients receive timely care. Requirements

  • High school diploma (required)
  • Prior home health scheduling experience (required)
  • Strong organizational and multitasking skills
  • Ability to work in a fast-paced, team-driven environment
  • Excellent communication and problem-solving abilities

Nice To Haves

  • Experience in home health administration (preferred)

Responsibilities

  • Scheduling patient visits in accordance with physician orders and care plans
  • Coordinating clinician schedules to optimize productivity and coverage
  • Managing daily scheduling changes, call-outs, and urgent visit needs
  • Answering and triaging incoming calls to support clinical operations
  • Tracking medical supplies and supporting RAP/EOE processes
  • Maintaining accurate scheduling and documentation within the EMR
  • Supporting the clinical team with administrative tasks as needed
