Concierge Home Care is seeking a Clinical Team Support professional, also known as a Scheduling Coordinator, for their Home Health division in Sarasota, FL. This role is crucial in supporting the clinical team by ensuring efficient scheduling of clinicians and timely patient care. It is a fast-paced, team-oriented position for individuals who excel in organization, communication, and making a positive impact. Requirements

• High school diploma (required)
• Prior home health scheduling experience (required)
• Strong organizational and multitasking skills
• Ability to work in a fast-paced, team-driven environment
• Excellent communication and problem-solving abilities

Responsibilities

• Scheduling patient visits in accordance with physician orders and care plans
• Coordinating clinician schedules to optimize productivity and coverage
• Managing daily scheduling changes, call-outs, and urgent visit needs
• Answering and triaging incoming calls to support clinical operations
• Tracking medical supplies and supporting RAP/EOE processes
• Maintaining accurate scheduling and documentation within the EMR
• Supporting the clinical team with administrative tasks as needed
