Requirements

  • Associate degree in Nursing or higher required
  • Active, unrestricted RN license in state of practice
  • Minimum 2 years of Home Health or Hospice RN experience
  • Experience with Microsoft Office and ability to learn enterprise systems such as Workday
  • Reliable transportation, valid driver's license, and current auto insurance
  • Ability and willingness to travel within assigned territory
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

Nice To Haves

  • Previous supervisory or leadership experience preferred
  • Homecare Homebase experience preferred

Responsibilities

  • Oversee daily patient care operations and clinical staff assignments
  • Coordinate referrals, patient care needs, and care plan implementation
  • Ensure timely patient admissions and compliance with regulatory standards
  • Review and approve physician orders and plans of care
  • Conduct OASIS reviews and maintain quality and compliance standards
  • Coach, mentor, and support field clinicians
  • Participate in interviewing and hiring clinical staff
  • Monitor key branch performance metrics and partner with leadership on operational goals
  • Coordinate communication among clinicians, physicians, patients, and families
  • Support field visits as needed
