About The Position

Trillium Health Partners is one of Canada’s largest community-based teaching hospitals, serving the growing and diverse populations of Mississauga, West Toronto, and surrounding communities. This role is for a Clinical Team Leader in the Mental Health Program, specifically focusing on Child and Adolescent care. The position is permanent and full-time, based at Credit Valley Hospital. The role involves creating and sustaining an environment that promotes professional practice, teamwork, and effective communication. It also includes coordinating day-to-day operations, providing leadership presence, facilitating staff development, and supporting team accountability. The position carries a reduced allocation of clinical responsibilities, but also involves providing direct clinical care, completing assessments, developing treatment plans, coordinating follow-up care, and acting as a practitioner, consultant, and educator.

Requirements

  • Minimum five (5) years of recent experience working with children and adolescents experiencing mental health concerns
  • Minimum three (3) years of experience in a formal or informal leadership role within a healthcare setting
  • Master’s Degree in Social Work, Nursing, Counselling Psychology, Occupational Therapy, or equivalent, with current registration in good standing with the relevant regulatory college
  • Demonstrated clinical experience in an acute care mental health setting
  • Sound knowledge of mental health disorders, relevant legislation, and child protection frameworks
  • Strong assessment skills, including mental status examinations, psychosocial assessments, and suicide and violence risk assessments
  • Experience supporting discharge planning and care transitions
  • Strong ability to work effectively within a multidisciplinary team in a fast-paced and evolving environment
  • Demonstrated ability to manage competing priorities, maintain professional composure, and support team functioning under pressure
  • Excellent interpersonal, communication, and organizational skills
  • Commitment to timely, accurate, and thorough clinical documentation
  • Working knowledge of community mental health resources and services

Responsibilities

  • Create and sustain an environment that promotes professional practice, teamwork, and effective communication through mentoring, coaching, and role modeling of child and adolescent mental health expertise
  • In collaboration with the Manager and team members, coordinate and prioritize day-to-day operations, including staffing, scheduling, and resource utilization
  • Provide consistent leadership presence through regular rounding, monitoring, and responding to patient and family experience
  • Facilitate staff orientation and support ongoing professional development by identifying learning needs and connecting staff with appropriate resources in collaboration with Program Educators and the Manager
  • Support team accountability to the Manager with respect to patient care, professional conduct, and performance expectations
  • Participate in recruitment, onboarding, and performance evaluation processes for staff
  • Collaborate with members of the interprofessional team (inpatient, outpatient, and Emergency Department) to ensure coordinated, high-quality patient care
  • Conduct regular chart reviews, audits, and follow-ups to ensure adherence to clinical standards, documentation requirements, and organizational policies
  • Promote effective interpersonal relationships within the team by supporting respectful communication, coaching team members, and facilitating difficult conversations as needed
  • Support patient flow, including admissions, discharges, and transitions, through transparent, equitable, and standardized processes
  • Demonstrate a calm, organized, and solutions-focused approach to leadership, particularly during periods of change, complexity, or increased demand
  • Provide direct clinical care, demonstrating accountability for clinical decision-making and the provision and coordination of patient care
  • Complete comprehensive assessments and develop treatment plans, including documentation of progress notes and discharge summaries
  • Coordinate follow-up care and connect patients and families with appropriate community and system resources
  • Act as a practitioner, consultant, and educator to patients, families, and members of the healthcare team
  • Contribute to clinical teaching, mentoring, and supervision of students and participate in interprofessional education opportunities
