About The Position

The Clinical Support Specialist is responsible for providing day-to-day support to care team (including RNs, NPs, Pharmacists, and Social Workers) and providing care coordination for patients. This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office. Requirements

  • Requires a H.S. diploma or equivalent and a minimum of 1 year experience or any combination of education and experience which would provide an equivalent background.

Nice To Haves

  • Understanding of Medicaid/Medicare coverages of DME and supplies strongly preferred.
  • Certified Nursing Assistant, Medical Assistant or Licensed Practical Nurse preferred.

Responsibilities

  • Assist Care Managers with administrative discharge planning activities across inpatient and outpatient settings.
  • Monitor and track discharge planning milestones, barriers, and follow-up actions.
  • Procures supplies and equipment in a timely manner to support patient independence in the home.
  • Supports order, delivery, and receipt of Durable Medical Equipment (DME) and medical supplies as identified by patient care team.
  • Facilitates care coordination with external medical providers and specialists.
  • Identifies and schedules referrals with the interdisciplinary care team.
  • Assists patients with scheduling visits with interdisciplinary care team members, as necessary.
  • Assists with and provides non-clinical activities related to HEDIS expectations as determined by quality management leadership.
  • Manages inbound and outbound inquiries and document all correspondences within relevant systems timely.
