About The Position

The Clinical Supervisor promotes an environment conducive to the delivery of quality patient care in hospice home care. This role involves assisting with chart audits, identifying trends, and ensuring timely correction of charting deficiencies. The supervisor provides direction to staff, monitors productivity, and participates in staff orientation and development. They are responsible for hiring, coaching, and terminating direct reporting staff, as well as coordinating on-call coverage and collaborating with management on process improvement. The position also involves conducting tracers/joint visits, ensuring accurate data collection (OASIS or Hospice Item Set), and working with operational staff to maintain patient and referral relationships. Additionally, the Clinical Supervisor establishes and maintains quality standards, develops collaborative relationships with customers and community organizations, and performs human resources responsibilities for their team, including performance reviews and recommendations for hiring and corrective actions. They are responsible for adhering to the organization's Code of Ethical Conduct and ensuring compliance with applicable policies, regulations, and laws.

Requirements

• Registered Nurse license issued by the state in which the team member practices, or Physical Therapist license issued by the state of Wisconsin, or Occupational Therapist license issued by the state of Wisconsin, or Speech-Language Pathology license issued by the state of Wisconsin
• A valid drivers license issued by the Division of Motor Vehicles.
• Bachelor's Degree in Nursing, or Bachelor's Degree in Physical Therapy, or Bachelor's Degree in Occupational Therapy, or Bachelor's Degree in Speech Pathology.
• Typically requires 3 years of experience in clinical nursing or rehabilitation.
• Must have thorough understanding of home health or hospice regulations, reimbursement and care requirements.
• Strong communication skills.
• Demonstrates an ability to organize, plan and delegate work effectively an efficiently.
• Demonstrates an ability to resolve conflict, motivate staff and interact with superiors, peers and subordinates in a direct and professional manner.
• Demonstrates an ability to solve problems and make independent judgments and decisions based on experience and good judgment.

Responsibilities

• Promotes an environment conducive to the delivery of quality patient care.
• Assists with chart audits and assures action is taken.
• Identifies trends in audits and communicates results to Manager and team.
• Assures field supervisory visits are accurately completed according to agency policy as necessary.
• Provides direction to the staff with clear responsibility for the coordination of resources to efficiently support day to day operations.
• Monitors and assures staff productivity according to established targets on a daily basis.
• Assures timely correction of charting deficiencies that impact revenue cycle and clinical operations by field team members.
• Participates in the orientation of staff through on-going direct teaching, coaching, observation, review and precepting of staff.
• Assesses educational needs of clinical staff and assists in development and facilitation of in services for staff.
• Participates in Field Team Interview/Hiring and Selection.
• Hires, coaches and terminates direct reporting staff.
• Coordinates and ensures 24 hour availability of field staff services and rotates on call coverage with Manager.
• In collaboration with the Manager, conducts ongoing process improvement.
• Conducts tracers/joint visits and provides on-going feedback to staff.
• Ensures accurate, timely and thorough OASIS or Hospice Item set data cycle time.
• Works collaboratively with operational staff and other leadership to ensure growth and maintenance of established patient and referral relationships.
• Establishes and maintains quality standards, continually evaluating workflow and processes.
• Develops collaborative and cooperative relationships with customers, physicians, community organizations and entities of Advocate Aurora Health at Home.
• Performs human resources responsibilities for staff which includes coaching on performance, completes performance reviews and overall staff morale.
• Recommends hiring, compensation changes, promotions, corrective action decisions, and terminations.
• Responsible for understanding and adhering to the organization's Code of Ethical Conduct and for ensuring that personal actions, and the actions of employees supervised, comply with the policies, regulations and laws applicable to the organization's business.
