Requirements

  • Current California RN license in good standing.
  • Graduate of an accredited nursing program.
  • Minimum of 2 years of management or supervisory experience in a healthcare setting; home health experience preferred.
  • Current CPR certification.
  • Strong clinical judgment and organizational skills.
  • Excellent verbal and written communication skills.
  • Ability to effectively supervise, guide, and collaborate with clinical staff.

Nice To Haves

  • BSN preferred.
  • Home health experience preferred.

Responsibilities

  • Receive and review patient referrals and determine appropriate home care needs and disciplines required.
  • Assign appropriate clinicians to patient cases.
  • Review and evaluate patient cases, clinical services, and documentation to ensure quality care.
  • Provide guidance and support to clinicians to promote effective performance and quality patient care.
  • Review patients' diagnoses, medications, procedures, and clinical progress.
  • Assist clinicians with establishing therapeutic goals, setting priorities, and developing individualized plans of care.
  • Participate in case conferences and facilitate coordination of care among team members.
  • Conduct quarterly clinical record reviews and communicate findings and recommendations to leadership.
  • Assist with the screening and interviewing of new clinical personnel and provide recommendations for employment.
  • Assist with the orientation and onboarding of new clinical staff.
  • Support the planning and implementation of in-service and continuing education programs.
  • Assist with the development, revision, and implementation of policies and procedures.
  • Promote excellent customer service and a positive patient-care experience.
  • Participate in community and public relations activities.
  • Perform other duties as assigned by the Clinical Director.
