About The Position

The emphasis of the position is to assist the Site Manager with the day-to-day oversight of the clinical operations as well as daily patient care. Furthermore, this position also handles day-to-day patient care within the setting of a primary care health center under the supervision of the Site Manager and provider.

Requirements

Excellent verbal and written communication skills, including ability to effectively communicate with internal and external customers
Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service
Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
Graduate of LPN or RN Accredited Program
Must maintain appropriate state licensure

Responsibilities

Supervise and manage the clinical operations of the site in collaboration with the Site Manager, including maintenance of supplies, facility, regular hours, consistent staff, staff schedules, lab services, patient charts, follow-up logs, patient education and counseling services, patient education materials, staff continuing education, new employee training, and scheduling.
Assist the Site Manager with the facilitation of communication among staff and administration, including regular staff meetings and participation in employee evaluations.
Assist the Site Manager with monitoring of services provided by nursing staff, including patient flow, time management, patient courtesy, quality of patient education, and current licensure.
Assist the Site Manager with the general supervision of office staff.
Assist the Site Manager with the monitoring of services provided by adjunct service staff (Nutrition, security, housekeeping, etc.), including time management, professionalism, patient courtesy, supplies, reliability, and thoroughness of work.
Delegate responsibilities as appropriate.
Participate in the interview/hiring and orientation process of future staff.
Initial triage of patients regarding urgency of presenting problems.
Locate patient in examination room and make available to the provider all medical records of the patient and any supplies or instruments that may be required by the provider.
Collect and record complete first order data (history and basic vital signs) before a patient is presented to the provider.
Assist provider as required in treatment or examination of patient.
Perform authorized lab tests and record information in designated files.
Perform EKG tests as required.
Keep all rooms supplied to needs of provider and staff.
Check rooms for cleanliness after each patient visit.
Maintain check on health center supplies and list items to be purchased.
Schedule diagnostic test for patients as per provider’s order as needed.
Provide instruction to patients in areas of medications, diet, health maintenance and diagnostic tests.
Record and observe provider orders.
Prepare specimens for outside lab test.
Make notations in EHR.
Ensure patient comfort and well-being while at health center.
Assist provider with procedures as needed.
Administer medications to patients according to provider orders.
Prepare sterile materials for use as needed.
Dispose of expendable material according to sanitation guidelines.
Perform nursing tasks such as dressing changes and ear irrigation as ordered by provider.
Identify family and environmental patient problems, and work with local health agencies and professionals in their solution.
Apprise the county health department by standard reporting procedure of any patient problem having public health implications.
Assist in care coordination which may include obtaining test and referral results and communication with various entities including but not limited to community organizations, health plans, facilities and specialists.
Proactively address needs of the patients and families through the concept of population management.
Effectively communicate with all patients with specific attention to the vulnerable patient population.
Provide self management support in the form of patient coaching and motivational interviewing when appropriate.
Acknowledge the importance of a team approach when providing patient care.
Communicate and coordinate effectively when providing patient care.
Actively participate in the medical home care team which is comprised of clinical and non-clinical working together towards a common goal: delivering quality care to our patients.
Maintain compliance with the organization’s confidentiality policy in accordance to the Health Insurance Portability and Accountability Act (HIPAA).
Maintain compliance with all company policies and procedures.
Other duties as assigned.
