This position leads an interdisciplinary Youth Assertive Community Treatment (YACT) Team, modeling high-quality clinical services and applying servant leadership principles to prioritize the needs of the team and individuals served. The role fosters trust, collaboration, and empowerment, delivering person-centered, community-based wraparound services to individuals with serious mental health and/or substance use disorders, promoting self-directed, fulfilling lives while minimizing hospitalizations and external resource reliance. The Youth ACT Team specializes in working with clients between the ages of 14-21 who are experiencing the first episodes of their mental illness and assists with transitioning successfully into adult services as clinically indicated.
