About The Position

Amazing Care Home Health Services is seeking a passionate RN Clinical Supervisor to lead, support, and elevate their Skilled Nursing team in the Westminster area. This role is key in ensuring high-quality, patient-centered care by guiding the team, managing clinical operations, and serving as a resource for staff and patients. The position involves overseeing patient care across the continuum, providing clinical leadership and mentorship, collaborating with interdisciplinary teams, and driving quality and compliance, including OASIS accuracy. The ideal candidate will adapt to a dynamic environment with a proactive, solutions-focused mindset.

Requirements

• Active RN license
• Proven experience in home health and OASIS assessments (required)
• Strong clinical judgment and the ability to work both independently and collaboratively
• Excellent communication skills and a team-first attitude
• A passion for developing others and delivering exceptional patient care

Nice To Haves

• Leadership experience in a supervisory or case management role prereferred

Responsibilities

• Oversee patient care across the full continuum: admissions, recertifications, routine visits, transfers, and discharge planning
• Provide clinical leadership, mentorship, training, and support to field clinicians
• Collaborate closely with case managers and interdisciplinary teams to ensure optimal outcomes
• Balance administrative oversight with hands-on clinical support, including occasional patient visits
• Help drive quality, compliance, and best practices (including OASIS accuracy)
• Adapt to a dynamic environment with a proactive, solutions-focused mindset
