About The Position

The overall responsibility of a Home Care/Hospice Clinical Supervisor is to provide leadership and support to field staff ensuring that Home Care patients receive excellent care that is supported by best practice in the most efficient and fiscally responsible manner. The ideal candidate is a proven self-starter with the ability to work autonomously with minimal supervision.

Requirements

Graduate of an approved school of nursing with a BSN, ADN with completion of a BA/BS or BSN within 3 years, or graduation from an approved school of Physical or Occupational Therapy
2 years Home Care or Hospice experience required
3 years clinical experience required
Licensed as Registered Nurse and in good standing with the Michigan Department of Licensing and Regulation or licensed as a Physical or Occupational Therapist in the State of Michigan and in good standing.
Possesses current Michigan motor vehicle license, ability to drive a car, and insured vehicle capable of transport to patient homes or other offices in various weather conditions.
Employees must be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella.
Employees must receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
