Clinical Staff Pharmacist Full Time: hybrid Summary: The Clinical Staff Pharmacist supports patient care by providing clinical expertise in symptom management, medication therapy, and cost-effective pharmaceutical practices. Reporting to the Senior Director of Pharmacy Services, this role partners closely with medical and clinical teams to ensure high-quality care for patients, particularly those with serious or end-of-life illnesses. The pharmacist also acts as a liaison with external pharmacies and helps drive safe, efficient medication use across the organization.

Requirements

  • Bachelor’s in Pharmacy required
  • Active Massachusetts pharmacist license (or eligibility)
  • Minimum 2 years in clinical pharmacy (acute, advanced, or terminal illness care) OR completion of PGY1/PGY2 residency/fellowship
  • Familiarity with EMR systems, pharmacy platforms, and Microsoft Office

Nice To Haves

  • Pharm.D. preferred
  • Hospice or palliative care experience strongly preferred
  • AppEnabler a plus
  • Strong communication skills
  • Understanding of hospice practices preferred
