Responsibilities

  • Reviews medication orders issued by physician or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
  • Ensures all orders are filled and labeled properly, including secondary labels.
  • Performs clinical functions as outlined by the Pharmacist in Charge.
  • Maintains the integrity and accuracy of the drug distribution and delivery systems.
  • Ensures the appropriate and safe utilization of high-risk medications and controlled substances.
  • Maintains patient profiles with current information on allergies, diagnosis, weight, current medication, and IV therapy orders.
  • Compounds and supervises the compounding of medications and intravenous solutions, including hazardous medications.
  • Reviews patient’s drug regimen for potential interactions, interferences, or incompatibilities and notifies physician or nursing staff as appropriate.
  • Assists in training pharmacy technicians and student pharmacists.
  • Demonstrates adequate skills in all forms of communication.
  • Works well with others in a spirit of teamwork and cooperation.
  • Exhibits specific attitudes, sense of ownership, and ensures departmental commitment to service excellence.
  • Adheres to established standards, policies, procedures, and protocols.
  • Supports the mission and vision of the facility.

  • Possesses sufficient knowledge, training, and experience to review, prepare, and assist the pharmacist in dispensing prescribed medications.
  • Maintains the sterility of parenteral medications where applicable.
  • Able to train and educate by setting the example, giving instruction, providing leadership, and generally raising the level of performance of others while on the job.
  • Demonstrates a proficient understanding of computers or has successfully completed training for applications used to support medication use processes.
  • Capable of using fundamental features of pharmaceutical computer applications to support patient care issues of moderate difficulty.

  • Monitors for therapeutic duplication or other unnecessary medications.
  • Interprets, monitors, and assesses patient's laboratory results as they relate to medication therapy.
  • Provides drug information to other healthcare professionals, patients, and their families.
  • Executes job responsibilities with an understanding of how output affects and impacts other areas related to own job area/team with occasional guidance.
