About The Position

Vanderbilt University Medical Center (VUMC) is seeking a Clinical Pharmacist to provide leadership in a specialized healthcare team and to the patient, ensuring optimal cost-effective drug therapy. This role involves developing specific pharmacotherapeutic goals, recommending treatment regimens, and creating monitoring plans tailored to individual patient needs. The clinical pharmacist is crucial in leading efforts to prevent and report medication errors and adverse drug reactions, contributing to the continuous improvement of institutional processes and practices to enhance medication therapy safety. This position operates on a schedule of four 10-hour shifts (12:45 PM – 11:15 PM) with every 6th weekend and holiday rotation. The pharmacist will deliver comprehensive pharmaceutical care to cardiovascular (CV) medicine/surgical ICU patients, including daily patient review, code response, pharmacy consults, transition of care support, patient education, antimicrobial stewardship, monitoring high-risk medications, and serving as a drug information resource. This position is eligible for relocation assistance and an evening shift differential.

Relevant Work Experience
1 year of experience
Bachelor's degree

Nice To Haves

Completion of an ASHP-accredited PGY1 residency.
Equivalent years of experience practicing in a relevant clinical practice setting.
Board of Pharmacy Specialties (BPS) certification in pharmacotherapy or other specialty certification.

Responsibilities

Reviews drug orders and/or prescriptions to ensure the safe, appropriate, and cost-effective use of medications.
Prepares and dispenses medications, provides drug information, and manages medication therapy in accordance with federal, state, and local regulations and hospital policy.
Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems.
Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas.
Supervises the activities of assigned support personnel and verifies the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients.
Delivers comprehensive pharmaceutical care to cardiovascular (CV) medicine / surgical ICU patients.
Provides daily patient review, code response, completion of pharmacy consults, support at transitions of care, patient education, antimicrobial stewardship, monitoring of high-risk medication therapies, and serving as a drug information resource for providers, nurses, and patients.
