About The Position

Kline Galland Benaroya Services is seeking a dedicated and experienced Clinical Nurse Specialist to join our Home Health team. This is an in-office role focused on supporting patients and clinicians through telephonic and virtual outreach, quality assurance, and interdisciplinary collaboration to maximize patient outcomes.

Requirements

  • Active RN or LPN in Washington State or RN or LPN Multistate license
  • Minimum 5 years of clinical experience, preferably in home health or a related setting
  • Strong clinical expertise in managing complex patient conditions
  • Exceptional communication and interpersonal skills
  • Strong analytical, problem-solving, and organizational abilities
  • Demonstrated leadership and mentorship skills
  • Commitment to patient-centered care and continuous quality improvement

  • Experience with telehealth platforms and EMR systems

Responsibilities

  • Provide expert clinical guidance to nurses, therapists, and aides through phone consultations, interdisciplinary team (IDT) meetings, and in-person interactions
  • Conduct telephonic and virtual patient assessments and follow-up visits
  • Monitor patient progress, manage care plans, and document interventions
  • Review clinical documentation and patient data to identify improvement opportunities
  • Implement quality initiatives and ensure compliance with regulatory standards and best practices
  • Conduct outreach calls to patients and families to assess satisfaction and quality of care
  • Serve as a clinical liaison between providers, patients, and caregivers to align goals of care
  • Collaborate with intake teams to review referrals and ensure smooth patient onboarding
  • Develop and oversee initial and recertification Plans of Care (POCs)
  • Ensure care plans are comprehensive, appropriate, and compliant
  • Identify patients who may benefit from telehealth/virtual services
  • Provide education and mentorship to clinical staff on best practices and new technologies
  • Lead or participate in interdisciplinary team meetings
  • Partner with leadership to promote high-quality patient care
  • Analyze clinical data and prepare outcome reports
  • Perform in-person visits on a PRN basis
  • Participate in Administrator On-Call rotation
  • Support additional initiatives as assigned by the Clinical Leadership Team
