About The Position
Kline Galland Benaroya Services is seeking a dedicated and experienced Clinical Nurse Specialist to join our Home Health team. This is an in-office role focused on supporting patients and clinicians through telephonic and virtual outreach, quality assurance, and interdisciplinary collaboration to maximize patient outcomes.
Requirements
- Active RN or LPN in Washington State or RN or LPN Multistate license
- Minimum 5 years of clinical experience, preferably in home health or a related setting
- Strong clinical expertise in managing complex patient conditions
- Exceptional communication and interpersonal skills
- Strong analytical, problem-solving, and organizational abilities
- Demonstrated leadership and mentorship skills
- Commitment to patient-centered care and continuous quality improvement
Nice To Haves
- Experience with telehealth platforms and EMR systems
Responsibilities
- Provide expert clinical guidance to nurses, therapists, and aides through phone consultations, interdisciplinary team (IDT) meetings, and in-person interactions
- Conduct telephonic and virtual patient assessments and follow-up visits
- Monitor patient progress, manage care plans, and document interventions
- Review clinical documentation and patient data to identify improvement opportunities
- Implement quality initiatives and ensure compliance with regulatory standards and best practices
- Conduct outreach calls to patients and families to assess satisfaction and quality of care
- Serve as a clinical liaison between providers, patients, and caregivers to align goals of care
- Collaborate with intake teams to review referrals and ensure smooth patient onboarding
- Develop and oversee initial and recertification Plans of Care (POCs)
- Ensure care plans are comprehensive, appropriate, and compliant
- Identify patients who may benefit from telehealth/virtual services
- Provide education and mentorship to clinical staff on best practices and new technologies
- Lead or participate in interdisciplinary team meetings
- Partner with leadership to promote high-quality patient care
- Analyze clinical data and prepare outcome reports
- Perform in-person visits on a PRN basis
- Participate in Administrator On-Call rotation
- Support additional initiatives as assigned by the Clinical Leadership Team
