About The Position

Trustbridge Hospice, a part of Empath Health, is seeking a compassionate and patient-focused Clinical Social Worker MSW to join their team. This full-time position works Monday through Friday 8 a.m. – 5 p.m and one weekend every 6 weeks. This role covers clinics/homes in North Broward County (Coral Springs/West Boca). In this important role, you will provide essential psychosocial assessments and interventions that help patients and families navigate end-of-life care—directly enhancing the quality, dignity, and comfort of their hospice experience. Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community-based nonprofit, they provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

Requirements

Degree in Social Work or a related degree such as Psychology. Masters preferred.
One year of experience as a Social Worker. In a hospice setting is preferred.
Ability to work effectively and flexibly as part of a collaborative interdisciplinary team.
Strong written and oral communication skills

Responsibilities

Provide psychosocial assessment and intervention for patients and families within his or her scope of practice as a member of the interdisciplinary team
Performs duties that include assessment, intervention, care planning, outcome evaluation, documentation and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in the delivery of hospice care to the patient and family.
